Use the checklist to prepare a conversation
Read the complete wording and examples on the CDC page. These checklists organise observations; they are not a diagnostic test. This page does not calculate a leap date from a birthday or due date. The developmental-leap definition explains that distinction.
A simple observation template
Copy these headings into a paper or digital note. You do not need an account or an app to use them.
- Checklist and age: which source you were reading.
- What I saw: the action, setting and approximate date.
- What I am unsure about: something not yet observed, a change or a lost skill.
- My question: what you want the care team to help clarify.
Fictional example: “Six-month checklist; reached for the rattle during floor play on Sunday. I am unsure how to describe another item and want to ask about it.” This is an example of a note, not evidence that all milestones are met.

For context at those ages, read four-to-six-month skills or six-to-twelve-month skills. The appointment checklist helps keep your main question easy to find. Seek advice about a missing or lost skill or another concern without waiting to complete every entry.
