Write the question before collecting more numbers
Choose the issue you most want to discuss and put it first. “What information would help us understand these changes?” gives the conversation a purpose. If there are several concerns, list them and ask how to prioritise the visit.
The American Academy of Pediatrics recommends reviewing records and questions before a child’s visit, taking notes and clarifying next steps Source 1. This worksheet is Cocoon’s own preparation aid. It does not assess symptoms or decide whether it is safe to wait for an appointment.
Bring a few observations with their limits
Choose dated examples that relate to the question. Say who observed them, which part of the day was recorded and whether a time is approximate. Keep original units with any measurements, and bring requested clinical documents rather than copying them from memory.
For example, “three evenings recorded; the weekend was not logged” tells the clinician more about the evidence than “sleep is always different.” Our record-confidence map explains how to label observations, estimates and gaps.
Leave room for the answer and the next step
During the visit, write the explanation in words you understand. If a term or instruction is unclear, ask for it to be explained or written down. Ask what should happen next, who will arrange it and how you will receive any results or follow-up information.
Use the clinician’s official care instructions for treatment details. This sheet has no prefilled medicine instructions, doses or symptom thresholds. If a note you wrote differs from the care plan, check with the clinician rather than guessing which version to use.
Close the loop after the visit
Review the sheet alongside the official after-visit information. Mark tasks that need arranging, and share the relevant instructions with the caregivers who need them. Keep the filled sheet private because it can contain health and contact information.
If you are worried about an urgent health problem, seek appropriate medical help instead of waiting to finish a worksheet. For broader preparation, read what to bring to a baby’s health appointment. The downloadable sheet is the writing space that accompanies that guidance.
